ITEMS SAFETY INSPECTION CHECK FOR IONIZING RADIATION

1.  Are radioactive isotopes used on site? If so are personnel aware of the dangers?
2.  Are all the required personnel carrying dosimeters?
3.  Is the storage area regularly surveyed and the results below 7.5 uSv /hr.
4.  Is the labeling of the storage area sufficient?
5.  Is the labeling of the source containers clear?
6.  Are sufficient barriers/warnings/flashing lights in place prior to exposing a source?
7.  Is the barrier monitored to be less than 7.5 uSv/hr?
8.  During exposure is the barrier permanently manned?
9. Have appropriate safety and environment talks been given on site? Have all the required personnel attended the talk?
10.  Are records kept of the location and condition of all radioactive sources?
